Signing Guide for a Scottish Continuing Welfare Power of Attorney

The Scottish Continuing Welfare Power of Attorney is a powerful document but in order to be in order to be valid must be signed and witnessed correctly and properly registered. The following guide gives important information on how this can be achieved and advice on how to sign your power of attorney.

Important Notes
It is vital that your Scottish Welfare Power of Attorney is executed in the presence of your GP.
Your document must be completed in the order described below.
Step One
Turn to page 4/4
Sign your usual signature on the line indicated in front of your witness
Complete the Date of Signing section in the format dd/mm/yyyy. For example, 17/03/2025.
Fill in the Place of Signing section with the location you are signing your document(s). For example, ‘Kilmarnock’.
Your witness must now complete their section, they must
Sign their usual signature on the line indicated.
Print their full name on the line indicated. For example, ‘Andrew Bruce Macdonald’.
Complete their full address in the appropriate section including their postcode.
We advise that you use your GP as your witness.
Step Two
Turn to the second to last page, the page after page 4/4.
In the box labelled ‘Insert date subscribed’ write in the date used in Step One.
Step Three
Turn to the final page in the document. If necessary give your GP the ‘Notes for GP’ pages supplied.
Your GP will satisfy themselves that they have knowledge of you and that your have the mental capacity to understand the nature and extent of your Power of Attorney.
Your GP will then sign where indicated.
Your GP will then complete the section below their signature, including their full name, and address with postcode.
Your GP will then add the date on the final line using the format dd/mm/yyyy. For example, 17/03/2025
Your Continuing Welfare Power of Attorney is now complete, ready for registration.
